Alternative Care (AC)
The Alternative Care (AC) Program is a state-funded program that supports certain home and community-based services for older Minnesotans, age 65 years and over, who are at risk of nursing home placement and have low levels of income and assets.
The basic eligibility for AC includes:
- Age 65 years and older
- The person’s income and assets would be inadequate to fund a nursing facility stay for more than 135 days
- Choose to receive community services instead of moving to a nursing home
- No other funding source is available for the community services
- Assessment completed by the Long Term Care Consultation (LTCC) screening team
- Found to need the level of care provided at the nursing facility level
Basic services that are covered by AC waivers include:
- Adult Day Care / Adult Day Care Bath
- Assisted Living / Customized Living
- Case Management
- Consumer Directed Community Supports (CDCS)
- Foster Care
- Home Delivered Meals
- Home Care (Home Health Aide & Skilled Nurse Visit)
- Homemaker
- Modifications and Adaptations
- Respite Care
- Specialized Equipment and Supplies
- Transportation
