Brain Injury (BI)

The BI program funds home and community-based services necessary as an alternative to institutionalization for persons under 65 years of age who are chronically ill or medically fragile and who would otherwise require the level of care provided in a nursing facility or neurobehavioral hospital. 

Eligibility - to be eligible for the BI Waiver, a person must choose the BI Waiver and meet all of the following criteria:

• Under 65 years of age at the time of opening to the waiver
• Eligible for Medical Assistance
• Certified disabled by Social Security or the State Medical Review Team (SMRT) process
• A BI Waiver Assessment and Eligibility Determination completed though MN Choice Assessment
• Diagnosed with one of the following documented primary or secondary diagnoses of brain injury or related neurological condition that resulted in significant cognitive and significant behavioral impairment:  Acquired brain injury including traumatic brain injury that is not congenital, degenerative or genetic disease where cognitive impairment is present, becomes symptomatic on or after the person’s 18^th birthday and is not congenital
• Able to function at a level that allows participation in rehabilitation
• In need of a service that is only available through the TBI Waiver or requires higher level of service than is available through other waivers due to cognitive and behavioral impairments
• Meets nursing facility level of care criteria for TBI-NF (Nursing Facility) or Neurobehavioral Hospital Level of Care criteria for TBI-NB (Neurobehavioral).
