Minnesota Senior Health Options (MSHO)

This is a program offered to seniors who are 65 years of age or older, living in the community or in a nursing home, eligible for Medical Assistance and eligible for Medicare Parts A and B.  MSHO includes Medicare, Medical Assistance, Prescription Drug Coverage and Elderly Waiver Services (if eligible). 

Des Moines Valley Health and Human Services contracts with UCare Minnesota and Blue Plus health insurance companies to provide county care coordination. 

The care coordinator’s role is to develop a relationship with the MSHO member, perform home based assessments, develop and maintain a collaborative plan of care with the MSHO member and the primary care clinic, facilitate an integrated team approach to care delivery, facilitate referrals and authorizations for services and coordinate benefits including the plan of care.
